Clinical RN Navigator - Heart Failure Cardiac Services

Virtua is home to the only LVAD program in South Jersey, delivering comprehensive, patient-centered care spanning evaluation, implantation, and long-term management.
Collaborate closely with our LVAD team, the first program of its kind in the community
Provide education and support to hospitalized heart failure patients
Engage in ongoing professional development with extensive opportunities to expand expertise in advanced heart failure care
Minimum of 2 years of experience caring for heart failure patients in either an inpatient or outpatient setting
Bilingual Spanish skills are a plus
Outpatient clinic hours: Monday–Friday, 8:00 AM – 4:30 PM
No weekends or major holidays
This is a unique opportunity to contribute to a highly specialized program while supporting patients across the continuum of heart failure treatment.
Summary:
Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Virtua and in the community.
You will have the opportunity to join a growing and dynamic heart failure program that offers excellent multi-disciplinary care to heart failure patients in our 5-hospital system.
Virtua Health is the first LVAD program in South Jersey and we care for patients along the continuum of heart failure from stage A to stage D disease and offer therapies like CardioMems that have been shown to help minimize readmission for heart failure.
The navigators work with autonomy at their hospitals to provide education to patients, families and hospital staff and to improve heart failure-related metrics at the hospitals. They are a critical bridge between the acute inpatient care and the outpatient world for our heart failure patients.
Position Responsibilities:
Serves as an essential link between patients and all other care providers. Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies. Adheres to established productivity benchmarks.
Facilitates appointments for consults and support services within established service standards, including social workers, dietitians, physician, etc. Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient care management resources.
Actively participates in multidisciplinary planning conferences.
Develops, plans and presents patient education programs and tools.
Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, etc.)
Works to collect data, track outcomes, and support strategic planning processes. Delivers reports as requested to Virtua committees and other groups which demonstrate outcomes and performance improvement activities.
Position Qualifications Required / Experience Required:
Experience in specialty (Cardiac and Heart Failure) required.
Microsoft Office competency required.
Database or registry software experience preferred.
Requires problem solving, decision making & critical thinking.
Requires excellent leadership, organizational, written & verbal communication & excellent interpersonal skills.
Must be able to work in a self-directed environment, with an ability to work with and lead teams.
Excellent presentation skills.
Ability to implement professional and community-based education programs.
Required Education:
BSN preferred
Training/Certifications/Licensure:
Active NJ Licensed RN.
BSN and OCN preferred.
Annual Salary: $79,719 - $123,934 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
