Full-Time Nurse Practitioner – Home-Based Palliative Care

Virtua Medical Group is seeking an experienced, compassionate, and dedicated Nurse Practitioner to join our Home-Based Palliative Care Team.

This is an exceptional opportunity for a clinician who is passionate about delivering high-quality, patient-centered care in the comfort of patients' homes and has experience caring for individuals with serious, chronic, or life-limiting illnesses.

About the Role

Palliative care is centered on improving quality of life for patients and their families facing chronic, complex, or serious illness. Our goal is to provide comprehensive medical, emotional, and supportive care that relieves symptoms, enhances comfort, and helps patients remain safely and comfortably at home whenever possible.

This position focuses on providing palliative care through home visits across the community, allowing clinicians to build meaningful relationships with patients and families in their own environment. The Nurse Practitioner will work collaboratively with physicians, nurses, social workers, hospice partners, and other interdisciplinary team members to develop individualized care plans that align with each patient's goals and values.

We are particularly interested in candidates with experience in:

• Palliative Care
• Hospice Care
• Home-Based Primary Care or House Calls
• Skilled Nursing Facility (SNF) or Long-Term Care settings
• Management of patients with advanced chronic illnesses and complex symptom needs
• Goals-of-care and advance care planning conversations

The ideal candidate is comfortable working independently in the field, passionate about caring for patients where they live, and skilled in supporting patients and families through complex medical decisions and transitions of care.

Compensation

The starting salary for this position is $128,125 annually.

The actual compensation package may vary based on factors including, but not limited to, the applicant's experience, internal equity, and alignment with market data.
