Transplant Clinical Pharmacy Specialist Fulltime - OLOL

Provides pharmaceutical care according to professional standards and practices and complies with all Federal and State regulations in the practice of Pharmacy. The Transplant Clinical Pharmacy Specialist is responsible for meeting CMS Transplant requirements by providing direct patient care at the Virtua Center for Organ Transplantation clinic at Virtua Our Lady of Lourdes, in addition to inpatient transplant coverage component for liver, kidney, and pancreas. This position is the second Transplant Clinical Pharmacy Specialist in this area.
Position Responsibilities:
• Evaluates medication prescribing patterns and identifies opportunities for improving drug therapy.
• Coordinate Drug Use Evaluation, Adverse drug Reaction Reporting activities.
• Provide education to patients/families, and other health care practitioners.
• Participate in medication safety initiatives in collaboration with the Pharmacy administration.
• Works within key sub-specialty areas to develop and maintain specific routine monitoring criteria for these patients.
• Represents the Pharmacy Dept. in system wide and divisional committees as needed
• Preceptorship for Transplant rotation for the Virtua PGY1 Pharmacy Residency program
Required Education:
Graduate of an accredited College of Pharmacy with Doctor of Pharmacy (PharmD) degree or BS and comparable clinical experience.
PGY2 Transplant Preferred
Training/Certifications/Licensure:
Active New Jersey pharmacy license.
Annual Salary: $120,981 - $199,417 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
