Registered Nurse (RN) – Home Health Care - Full Time
Pay Range: Approximately $43.27 – $58.68 per hour (based on years of experience)

Job Summary:
Provide skilled nursing care in patients’ homes following physician orders and a comprehensive plan of care. Serve as the clinical lead during home visits, perform assessments, and coordinate care across the care team.

Qualifications:

• Current and unrestricted New Jersey RN license.
• One to two years of recent acute care or community health experience.
• Baccalaureate degree in nursing preferred.

Clinical Nurse Specialist – Home Health (Full Time) — WOCN Required

Pay Range: Hourly rate range approximately $44.31 – $60.11 (based on years of experience)

Job Summary:
Serve as an advanced nursing practitioner focused on wound, ostomy, and continence care within the Home Health division. Utilize evidence-based practice to assess, plan, implement, and evaluate complex patient care in home settings. The Clinical Nurse Specialist also supports outcomes improvement, patient education, and interdisciplinary care coordination across the Home Health team.

Qualifications:

• Valid New Jersey RN license.
• Current board certification as WOC (ET) nurse (CWOCN) or enrollment in an accredited WOCN education program with certification obtained within 12 months of hire.
• Minimum of one year of recent clinical nursing experience (hospital or community health).
• BSN preferred.

Licensed Practical Nurse (LPN) – Home Health Care (Full Time)

Pay Range: Hourly rate approximately $25.24 – $37.86 (based on years of experience)

Job Summary:
Provide foundational nursing care in the home setting under the supervision of an RN and according to the plan of care. Administer treatments and medications as ordered, educate patients and families, and contribute to documentation that supports clinical outcomes.

Qualifications:

• Current and unrestricted NJ LPN license.
• Graduate of an accredited LPN program.
• Minimum of one year of experience as an LPN preferred.
• Valid driver’s license and personal vehicle required due to travel between patient homes.

Hourly Rate: $25.24 - $60.11 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
