*Please note that this is NOT a remote or WFH position.

Virtua has introduced a new and exciting form of Nursing – “Hospital from Home” (H@H).  The Hospital at Home program is a technology driven care model that allows our patients to receive med/surg level of care safely in their home. This can provide you with the personal interaction that you are missing on the floor.

Each patient is selected by a criterion that is set forth by a panel of Physicians, Nurses and Administrators.  Patients are identified either during a current admission or directly from the Emergency room. They are very excited to receive care in their home. Hospital at Home allows them to fully recover in their own environment.

H@H requires our nurses to make two visits per day (or more as needed) to assess our patients, deliver medications, provide education and other nursing interventions.  Mileage and all necessary equipment will be provided.  DME is determined prior to discharge and is sent to the home.

Please visit Hospital at Home (virtua.org) for additional details regarding the program.
