Clinical Pharmacist Infectious Disease Fulltime - Voorhees

Provides pharmaceutical care including the dispensing of prescribed medications and other pharmaceuticals as needed according to professional standards and practices and complies with all Federal and State regulations in the practice of Pharmacy. Infectious Diseases clinical pharmacy services to include system-wide (5 hospital) antimicrobial stewardship program; expertise in the clinical use of antimicrobial agents and place in system formulary; collaboration with Corporate Infection Control; to advise and ensure compliance with Joint Commission Antimicrobial Stewardship Standards.
Advises/consults with health professionals, patients, and families regarding pharmaceutical therapy.
Position Responsibilities:
• Evaluates medication prescribing patterns and identifies opportunities for improving drug therapy.
• Coordinate Drug Use Evaluation, Adverse drug Reaction Reporting activities.
• Provide education to patients/families, and other health care practitioners.
• Participate in medication safety initiatives in collaboration with the Pharmacy administration.
• Participates in patient rounds with appropriate individuals including physicians, nurses and other health care providers.
• Works within key sub-specialty areas (ex. Infection Control, Antimicrobial Stewardship Committee, etc) to develop and maintain specific routine monitoring criteria for these patients.
• Represents the Pharmacy Dept. in system wide and divisional committees as needed. (including chair/secretary)
Required Education:
Graduate of an accredited College of Pharmacy with Doctor of Pharmacy (PharmD) degree or BS and comparable clinical experience. BS.
PGY-2 in Infectious Diseases
Training/Certifications/Licensure:
Active New Jersey pharmacy license.
Annual Salary: $120,981 - $199,417 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
